Ram 2500 stuck in 4th gear

Please help me figure out why my 2009 Dodge Ram 1500 4wd hemi is getting stuck in 4th gear.

The PCM sends a signal (at startup) to tell the Trans Relay to close, which then feeds 12V power to the trans. In my instance the shifter actually jumped out of the grooves between the shift forks.
